Here are the steps to follow:
Click Quizzes on the left page of your Canvas course.
Click the + Quiz button.
Enter a name and description for your quiz.
Select the Quiz Type (Graded Quiz, Practice Quiz, Graded Survey, or Ungraded Survey).
Select the Assignment Group (if applicable).
Select the options for Shuffle Answers, Time Limit, Multiple Attempts, etc. (if applicable).
Select the options for Assign to, Due Date, Available From/Until Date (if applicable).
Click Save or Save & Publish at the bottom of the page.
